I just wrote up a long response, but then lost it all when trying to post it, ahhh!

So in summary:

Write a script yourself using the FDF Toolkit:

Or use 3rd party GUI tools to route the data to your database. Two tools you should check out are ARTS PDF Form Manager and ARTS FDF Import.
