Topic: Creating PDFs with different page sizes

I have a custom app (written in vb.net and c#) that prints paper documents and creates PDF documents of different page sizes from predetermined forms. The page sizes are custom and vary from 8.5"X11" to 48"X48". The user assigns the printer (or Distiller if a PDF is desired) when initially creating the form. We have acrobat distiller installed on each user's machine in order to create the PDFs. Currently, in order to create a PDF with the correct page size, the user needs to manually go into the Distiller print driver (on their machine) before creating the PDF and change the height and width of the custom page size every time they want to change page sizes. We now need to be able to do this without the manual intervention.
Is there a way to do this? Possibly with either:
A) Change the height and width of the distiller printer driver programatically?
B) Create "virtual" Distiller printers, each with their own custom page size? The user can then select the correct "virtual" printer for each form by size.
