All right, I've seen that this is a very frequent problem that developers have when generating PDF files on the fly. And it looks like almost, if not, every time, the problem has to do w/ the xref table or the offsets within. I also learned that these entries have to be precisely 20 bytes long, so I made that change.

I'm still getting the rebuild message, so I took to a hex editor and started looking at my offsets at the precise position. Everything looked on queue.

I even went as far as to write a little (rough) program that will find the startxref, seek to the xref table, grab all of the offsets, seek to those offsets and spit out what it finds at that location in the file. Every one of the offsets starts with an "x 0 obj" .. seemingly correct.

So now I'm at a complete loss. There must be something silly that I'm missing or screwing up and I hope another set or two of eyes might be able to help me locate it.

I've attached the PDF file I'm generating (its a bare bones file) and I'm also attaching the source to the rough program I wrote to display the offsets if anyone is interested.
