To my knowledge there isn't a method in activePDF Toolkit for adding new
form fields. There is a new version of activePDF Toolkit on the horizons
though, so this could possibly include such functionality.
Otherwise, iSedQuickPDF has a method to allow you to add form fields ->
'NewFormField()'.
