Ensure you use the 'Acrobat Distiller' ppd file and give Acrobat 4 a memory lift - increase it to maybe 80,000 kb.

Additionally, there were issues with Acrobat 4.0 - though it's doubtful you'll be able to patch up to 4.05 now anyway.

I would download Acrobat Reader 5.1 (for OS 9) and then see if the image displays.
