Take a look at pdf-FormServer. This client, server or web-based solution allows you to save your forms to your local desktop, to your server or submit forms with content via web.

The client edition is only 35 $ and you can save as many forms as you want.

You can find a trial version at www.pdf-office.com (together with many other really great PDF tools)
