Basically we have staff that scans in Invoices from service providers. These PDFs are then emailed to Proj Mgrs who need to approve them. The approval requires filling out 5 separate lines indicating Client, Contract, Site, Amount and Approval. Basically I wanted to set up a graphic in Illustrator, create a PDF of it, open that PDF add form fields and Dig Sig fields, save the PDF as a stamp and have our staff simply stamp the Scanned in Invoice and email for approval. This isn't working as the stamp isn't saving the form fields. I don't want to add a separate page for this if I don't have to.

Any thoughts and your time are much appreciated.
