Topic: How to set the name of a PDF generated 'dynamically

When writing a PDF from a stream in PHP, JSP, etc., and the user views the PDF within a Browser, is there a means to explicitly set the name of the PDF file that is displayed in:
a) the save file dialog box that appears when the user clicks the "Save" icon to save a copy of the file to their local drive and,
b) within the dialog prompting the user to enter a password to access the file?
This applies to Reader, but I know Pro and Standard function the same way.
Currently when writing the stream to the browser, after the PDF is displayed if the user clicks the "Save A copy" button in the toolbar, the filename is set to "PDFoutput.jsp" - or whatever the page in the URL is. This also appears when the user is prompted to enter a password.
I think it would be fine even if the title where the default.
I know that if we were dealing with HTML, the filename can be set in the header, but that clearly doesn't apply here.
