An image mask is a 1-bit image, which is still an image, but unlike all other images, can be coloured. Also, it has no background colour; it only paints the foreground.
Nothing is wrong - these are normal parts of PDF. Some programs may use them for 1-bit images. A Photoshop EPS of a 1-bit image has the option "transparent whites" which effectively produces an imagemask when distilled.
