I am using Acrobat 6.01 and have a document with no security passwords assigned to it. When I try to add a form field (say a checkbox), it draws the outline but then no control appears. I can select an existing control and copy/paste it. But then I can not change the new copy's name. The origional was created on a max (4.x Acrobat). I have not ever had this problem with previous documents. Attached.
