Topic: Creating downloadable personalized PDF Files from a web page database

I am trying to create a web page that will allow our clients to download a personalized PDF file from a web-sight. I envision it to work like this a company would enter their name into a text field on the web page. Then from the web page the name would go into a database on our server and from that database the web page would then create a PDF file with the company name placed into it. Which would then be able to be downloaded and when the company downloaded the file and opened it, the PDF file would then be personalized for their company. For example: if you had a downloadable letter in a PDF format on your web-sight and on the web-page you had a text field were someone could enter there name in before they had permission to download the file. And after typing in their name and being allowed to download the file, when they would open the downloaded PDF their name would be in the "Dear John Doe," field. If you understand what I am trying to do I would appreciate any assistance that may be out there. Any suggestion as to how to accomplish this task would be appreciated. Thanks for your time!
