Topic: Programmatically inserting an annotation (image) into PDF

I am currently doing PDF development in C++ and I want to write a function that inserts an image as an "PDF annotation" into (say page one of) any PDF file.

My problem is that I cannot figure out how to convert an image file (say BMP/JPG/GIF/PNG etc) into the PDF "stream" to add to the internal objects to embed the annotation so that it shows up as an image when viewing the PDF (say in Adobe Acrobat Reader)

Can anyone point me to some available source code or tutorial on how to achieve this?

Basically I would like to give as input the name of the image file and that image file should be embedded into the PDF as an annotation which should show up when the PDF is next opened in a reader...
