I'm new to this forum and could use some guidance on best ways and with what software to do the following (all running on a web-based server):

1.) Field client input through through a web page in the form of 10 short fields of text.
2.) Allow the client to choose from a limited list of font choices and font sizes for each field of text.
3. Process this these text fields into predefined sections of a one page document. Note that some text fields need to be inserted into a section that puts that text on a predefined arc or curve. (If this virtual document has to exist, it might well be a postscript page.)
4. Display a proof of the final output page - this is presumed to be a pdf format page.
5. Recycle the client to reset any text and properties until they approve what the see in the proof.

I am attempting to attach a sample pdf to assist understanding how the final document might appear.
