Yes - Acrobat's plugin to Word has detected the structure (the linked
Table of Contents) in your Word file and has created PDF structure to
match. (Well, one can hope it matches, anyhow).

Nope! Why would you think that you need to? Alt. text is required for
images, not text.

These are simply a few of the "standard" tags available for use in
structuring PDF content. "TOCI" stands for "Table of Contents Item" -
to indicate a sub-part of a "TOC" (I prefer "ToC", but I digress).
