I'm having exactly the same problem with Word 2003 SP2 and Distiller 7.0.5.

I started off with a Word document which works, but now that I've edited it it doesn't do headers & footers in the PDF.

Printing to the Adobe PDF printer gets round the problem but the resulting PDF has no bookmarks. It looks like an Adobe/Word bug.
