Is there a way to make a text box that grabs information from a CSV file or Tab Deliminated file?

I have an Illustrator file that a PDF was created with. We want the background and some of the text to stay the same, but we would like to format it to take the information from our Outlook contacts and automatically insert them into the same spot with different names. There are about 200-500 names and it would take way too long to enter them all by hand. Any suggestions?

I do not know how to make a form and I was trying to do that as well.
