I successfully created a PDF form that emails the text field contents to a
my clients HR dept.

It looks like this when it is emailed to the HR dept. plain text email

first_name = Michelle
middle_name = R
home_address = 6162 College St
apt =
city = Jamesville
etc....(40 more fields)

What options do I have to create a more readable/friendly/workable

Can the form be emailed as a completed PDF so that 'graphically' it looks
like a typed in PDF?

Is there a third party service provider that can provide this functionality
for a monthly fee?

This is a small to medium client, so cost will be an issue.

Or, can this list point me to some resources/ie developers that can help
make a more readable version for the HR dept.

Thanks in advance for pointing me in the right direction...
