We currently ran into the same situation: The PDF is created from OmniPage as Image + Text. We could not tag the images/figures on the OCR'd PDF as OmniPage did not recognize the graphic zones.
Duff mentioned that "you are left with "hiding" the text as "Actual Text" for some convenient text-element (such as a period). Do you mean that we can put a '.' in the graphic zone such that it can be tagged as a figure?
Can you please explain a little bit more on this technique? Use Acrobat Pro or Acrobat SDK? We actually tried to use SDK to add some text element within the graphic zone. However, when we try Add Tags to Documents, it only taggs the text element we just added, instead of other elements in the page. In that case, we have to re-tag everything.
More generally, how the text layer is saved in a Image + Text PDF file?
