Adobe don't support such features on the command line, but you can do this with the Adobe SDK or you could get the print window handle and play with it in a program.
OR you could use our print tool Batch & Print Pro + PDF Addon Plus which can print as image.
NOTE: Adobe Reader 5.1 has that option in the main print window, we use that.

To use on command line do:
"Batch And Print" -Apdf"-U0|-s1|-e99999|-c1|-r0|-i1|-C0|-h0|-x0|-a0|-m0|-o0|-d0|-S0|-pEven and Odd Pages|-PLanguage Level 2|-lE:\Program Files\Adobe\Acrobat 5.0\Reader\AcroRd32.exe|-b0|-z0|-L0|-D0|-H0|-A0|-O0|-M22" c:\test.pdf -p -q
the -i1 parameter sets the print as image
