I use Acrobat Version 8 and it's simple. Open the pdf - click on Tools - Forms - Button Tool. Draw a button on the form. There should be a Button Properties box that opens.
On the Options tab, under Label: type the name of your button (like: Email)
On the Actions tab, in the Selection Trigger: box choose Mouse UP.
In the Select Action: box choose Submit a form. Click on Add.
In the Enter a URL for this Link: Type in mailto:jDoe@aCompany.org
Click on PDF The Complete Document
Click OK
Click on Close - you should see the email button.
Now click on Advanced, from the top menu, choose "Enable Usage Rights in Adobe Reader.
Now when the user clicks on send it will show up in an email.
