I am trying to create a form from Combo boxes and Text boxes. The first Combo box would be visible and all remaining fields would be hidden. The next line of Combo/Text boxes would depend on what the user chose in the first Combo box, etc.

I have created a form using Check boxes which works ok, but I feel the look of Combo boxes would be preferred.

I am obviously missing something that is preventing this from working. See attached.
