A client has asked me to put a 5 page application form on the website I'm building for them.

I've done the form and it works great locally and my client is really happy with it.

But as the application form contains a lot of personal information:- name, address, education & work history, medical history - I have the following concerns

1. If I put the form online and ask the user to fill it in there will there be a risk of identity theft?

2. How secure would it be to include details as sensitive as bank account details etc . . . (I'm sure the answer to that is not very but I need to ask anyway).

3. What would be best way to proceed? I normally do forms online with php but they normally only have 4 or 5 fields. This one has almost 30 so the pdf option makes my life a lot easier if I can use it online.
