Form XObjects are a bit like a group of objects. Not exactly the same, but perhaps that's the easiest way to think of them.
Some programs see Form XObjects as a single object, with no detail. Quite A Box Of Tricks did up to version 1.2f. Now, it looks inside the groups and works on the individual object. (Actually, it always did that for colour conversion, but not for Shrink or Info functions).
Where do they come from? Three sources
1. Distiller can make them when you have "OPI processing" on. You can switch this off.
2. Some design tools will export them. Especially InDesign. Place a PDF, it will probably export as a form XObject.
3. Some plug-ins make them. Quite A Box Of Tricks will make them when you flatten annotations, for instance. Quite Imposing will place pages as form XObjects under certain very specific situations. (Quite Imposing avoids this whenever it can to keep editability).
