There is a package from Heidelberg called ImageSmart Document Mastering 1.5 that includes Adobe Acrobat, Quite Imposing Plus, Stamp PDF and PDF Imageworks that may give what you are looking for. Quite Imposing and Stamp PDF gives you the flexibility to add annotations to your PDF file along with other features such as imposition, adding blank pages to name a few. PDF Imageworks gives you the ability to edit images within the PDF file allowing the user to change CMYK images to grayscale images and host of other features for images.
