You cannot refer to bookmarks.

You can refer to named destinations.

You can also refer to pages as in
http://www.quite.com/books/apu14.pdf#page=10

This is in the Acrobat 4.0 documentation, but inexplicably everything that might help deploy PDF files on the web has been removed from the 5.0 documents.
