You have to create a custom paper size in the
Distiller PPD. On Windows NT you do this by
selecting StartMenu>Settings>Printers, then
hilight the Distiller printer and select
File>Server Properties. The "Forms" tab allows
you to create a new paper size. Once the system
know about your custom size, the PostScript
driver can request the correct paper size and
distiller will create a PDF file based on this
size.
