You should implement a server based solution.
As Reader can not save your form with data you
will need to merge the form and data.
User fill the form, submit it to server and receives a
new pdf with form and data merged.

What probably Adobe is saying is that if your users
need to change or include form fields they will
need the full 5.0 suite.
