Has anybody encountered a problem with PDFMark code used in Framemaker 6.0 files and Acrobat distiller 4.0.5 to produce form fields. I've been trying to include the correct code to produce push buttons that will be used as navigation buttons (First, Prev, Next, and Last) in the final pdf file. I use variations of the following PDFMark code to produce the various push buttons:

[ /Subtype /Widget
/Rect [ XLL YLL XUR YUR ]
/T (next)
/FT /Btn
/Ff 65536
/H /P /F 1
/BS < /MK < /BG [ 0 1 1 ]
/CA (Next)
/RC (Go To)
/AC (Next Page) >>
/TU (Go to Next Page)
/DA (/Helv 10 Tf 0 g)
/Action << /Subtype /Named /N /NextPage >>
/Subtype /Link /ANN pdfmark

This works fine for the first occurrence of the button which displays and operates correctly. All the other buttons are invisible but are physically in the document and also operate correctly. If I double-click on the button with the Acrobat form tool, change nothing, and press OK the button it is then displayed correctly. It's also very odd that all the appearance
settings are correctly set when I check them with the form tool.
I'm keen to use push buttons rather than links so I can control whether or not they are printed.
