I in fact managed to find the solution with the help of Ivan Lee (ilee@boh.com) who had encountered the same problem some time ago. What was needed was the addition of the /NeedAppearances true flag included in the PDFMarker code of each button. I haven't managed to find this documented anywhere but an example of it's use can be found on Tomas Merz's web site (http://www.pdflib.com/pdfmark/index.html) in his "acroforms.eps" support file.
