To follow up...
/NeedAppearances true tells Acrobat/Reader to build the appearance of the
form field. If the file is saved using 'Save As', the built appearances are
saved with the file, and the key-value '/NeedAppearances true' is removed
from the AcroForm dictionary.
For fields constructed through the pdfmark operator, the /NeedAppearances
key is needed (initially); for fields constructed by the UI of Acrobat, it is
not.
Adobe recommends that all PDF be saved using 'Save As' to build the
appearances of any form fields; this reduces dislay time of the page because
the viewer does not have to build the appearance. This is especially
important for files being viewed within a browser.
