I've lost the train of thought for this thread, but I'll throw in another
two cents worth, since the topic is the distiller.

One thing that I recently learned is that even though
you set the distiller on 'Optimize for Fast Web View'
and you set /NeedAppearances true, distiller does not build the
appearances. I had always associated 'Optimize' with building the
appearances; these are, however, two separate issues. While
disiller can optimize, only Acrobat (through 'Save As') can replace
the code generated by the pdfmark with a bit stream that is the
appearance of the field.
