You might look at pre-defining your form fields etc. with PDFMARK
commands. Depending on the layout software you are using, you can put
them in "direct PostScript" sections, or place them on the document
using EPS files.
This approach gives you the possiblity to assemble the "smart"
document in the layout program and, after processing with Distiller,
you have your fields placed correctly.
Acrobat is not a layout program and should not be misused for that
(although I sometimes start with a blank page ).
You might also look at a forms design tool, and see if it supports your needs.
