Ken,
yes, this can be accomplished.
When you look at the FDF specification, you see the /APRef parameter
which allows to insert a graphic as icon of a button form field. And
I think that does indeed work with Reader. It could be that you might
first have to convert the image to PDF, but if your users have
Acrobat 5, this even should no longer be an issue.
