IAC is only designed to add text annotations. The forms API can also be used to add form fields. To add any other kind of annotation, so far as I know, you write a plug-in. This will require detailed study of the Cos objects and contents streams required by each type of annotation: there is no exposed API to the new kinds of annotations.
