Topic: Reader 5 doesn't show pdfmark navigation, 1 Attachments

Greetings. I've recently taken over a large codebase for a PDF-based cdrom that uses pdfmark commands to create navigational buttons (for home, zoom, next page, previous page, find, and print.)
The original postscript navigation code was written and tested for Acrobat Reader 3, and seems to have significant compatibility problems with Acrobat Reader 5. The main problem is that when opened in Reader 5, the navigational buttons are "invisible" but functional. I.e., the "Home" button is on the page, and responds if you click on it, but you can't see the rectangle or label text.
After spending two weeks studying postscript syntax and pdfmark documentation, and trying every minor adjustment to the code I could think of, I'm stuck. I'd greatly appreciate any help.
Are there any known issues with Acrobat Reader 5 related to the use of PDFMARK /ANN type navigation buttons? I have a hunch that the problem is in the layering of objects on the page, or in the syntax used for drawing the button rectangles... but I don't know the code well enough to find the problem.
I've attached a sample postscript file from the cdrom which includes the navigational code. When distilled, this code should result in a PDF page with a black navigation menu across the top of the page.
Thank you very much for your help,
