Topic: Damaged but being repaired' message

I'm writing an application that generates PDF, and various versions of the Reader all give me an error, "This file is damaged but is being repaired" when I open my output files.
In fact, I can't even get the simple sample file from the PDF specifications doc to load correctly. And it's not the xref table, because I've checked and rechecked that it's all in order. Some of the viewers I've used offer to correct the file, but they end up putting a bunch of junk in, and leaving the pre-existing elements as is, so it's impossible to determine what they found wrong.
Any ideas of why this might be happening/how to prevent it? I find ignoring the message pretty easy, but it might concern the user.
