Topic: Decrease documentsize with repetitive forms.

currently I'm working with Acrobat 3.5 for my employer. We are working on converting manuals from Word to PDF format with the intension of publishing on cd-rom. In this process I came to the following problem for which I hope you have an answer:
Let's say I have a document of about 50 pages. It has an index at the end and a table of contents in the beginning of the document. The idea is to insert buttons (named index and table of contents) that link to the corresponding pages. So I made 2 forms that did so, used 2 images made with Photoshop (size 4 Kb each) and duplicated the forms to every page in the document. Doing this the size of the document grew with about 50 * 2 * 4 Kb.
The image size is already reduced to the minimum, so I was wondering if there is an other way of saving precious space. Ways I can imagine are applying a template to the document containing the buttons as images and linking them to the corresponding pages. But the solution must applicable to a number of documents which will receive the identical fomat of buttons, so the linking must be done in the same way as duplicating forms...
I will be very pleased if someone could help me out here...
