To make a PDF File open with the Bookmarks pane visible, you need to set
(for each PDF file) the Open Settings. (File>Doc. Info>Open ...). In there
you will find settings for Initial View, select the 'BookMarks And Page'
Bookmarks and Thumbnails can be displayed at the same time (by dragging
their pane tabs to the centre of the screen), but you cannot control this
behaviour on the users machine.
To create Bookmarks like you suggest follow the procedure in the Acrobat (4)
Guide pages 229-238. (this guide is available from the Help Menu.
