You can't with PDFlib. First and foremost pdflib doesn't support
generating PDF form fields - so if that's a requirement, you're out of luck
with PDFlib.

None of the current HTML->PDF converters on the market support
conversion of forms, so that's not much of an option right now.

Your best bet is to either start with an existing HTML->PDF
converter such as HTMLDoc ( and extend it to handle
forms OR license a 3rd party library which can generate form fields and
then write the HTML parser and layout engine to go with it.
