Mark,

Sanface (http://www.sanface.com) make a Perl Script Interface to create PDF
files from HTML forms. If you want something more sophisticated then
perhaps check out DigiApp (http://www.digiapp.com) they have premade
libraries to assist in the generation of 'full featured' PDFs.

I'm not sure if PDFStore has some commercial apps listed but it's worth a
look (http://www.pdfstore.com)
