If you are writing this as a plug-in for Acrobat, it's pretty
easy. Just create a new content stream for each of the elements,
keeping in mind that they get drawn in the order that they are
listed. So put your hidden text first, then the image, then the
visible text.

I am pretty sure that you can NOT do this from OLE/COM, so
the only other option would be a 3rd party library.
