Here are some pointers for places to be looking...

1) Use the PDWordFinder API's for searching for strings.
2) Use PDPage and PDAnnote routines to create the annotation on the page
3) You will also need to do a bit of low level Cos work to get all
the dictionary entries correct - which means a good reading of the
PDF spec.
