there are a few possibilities.
If you are looking for a self-containing solution, your end users will need
Acrobat (either the full product or the upcoming Acrobat Business Tools).
Then you would have master documents where the individual pages are
specified as Templates. Then, you can use the Template functions in order
to put together your documents.
If you are in a web server environment, you can do similar, but have either
the templates used on the server, and/or use some commercial products, such
as FDFMerge from Digital Applications (http://www.digapp.com).
There are other server based systems around which can combine fragments
into a finished document.
