You can't. There are no functions to "query" the current matrix values or
get x,y info. You have to set the x,y yourself and keep track of text width
through font metrics. The operator list is in the PDF specification.
Examples are also in the Spec.

You must tell the viewer where to draw things. The viewer app doesn't
communicate back to the PDF document in any way.

This is why InDesign costs $700. The program logic to create good-looking
pages (in Postscript or in PDF) is extremely difficult and complex to
