I would recommend that each user get a copy of Acrobat Approval
($39) and then you build a form which connects using ADBC (ODBC for
Acrobat) to a local ODBC database (Excel, Access, whatever) and stores it
until they connect. At which point you have some process that then
transfers the data from the local DB to the remote one.
