PDFWriter is dead! It's no longer part of the standard
installation with Acrobat 5. Switch to using Distiller!
And when you switch to Distiller, you can use PDFMarks to add the
form fields to the document as part of your printing process!

You could using IAC/COM/OLE to Acrobat 5 and the new features
above. OR as noted above, you could have it down automatically as part of
the generation process when you switch to Distiller.
