This is the kind of thing you can do with a custom plugin written in C using
the SDK. You can traverse pages, elements within a page, etc. and make
permanent changes.

The PDF library (Adobe) is no longer being licensed, but would have cost you
$50K or so to start. You don't need it. PDFEdit is one of the layers in the
plug-in SDK. The SDK has enough to do what you need to do. Typically you
would only need more than that if you were doing PDF "read" and display
operations into a window of a custom app. If you need to rasterize and
display PDF elements, you should probably look into Ghostscript (an
open-source project).
