Prabath,

Generally spoken, you will use FDF (Forms Data Format) files to do this.

if you are in a webserver environment, you can either use your
database directly to create the FDF, or you can use the more
comfortable FDFToolkit.

If you are not in a webserver environment, there are no "standard"
direct ways. You can, of course create FDFs, and then load them into
the PDF form, but that would require a well-known environment (for
Reader; for a full version Viewer component, such as in the Business
Tools or in the Full Acrobat, this is not so critical).

I know about a solution which uses a custom made plug-in which makes
the ODBC interface available, but I am not completely sure if this
solution runs under Reader. In any case, feel free to contact me in
private about further information.

Hope, this can help.
