this is done with the PDFMARK operator. You will find the description
of this operator in the SDK documentation (

What happens, is that you have a special Postscript command which is
handled by Distiller only, and allows to have Distiller insert
annotations for example. In fact, this PDFMARK mechanism is also used
by the software itself to create links etc.
