It's a 2 x 3 matrix - [ a b c d e f ], where a & d are
scaling factors, e & f are translation factors and b & c are used for
rotation and skew. Again, this is all in the PDFspec...

They shouldn't make any difference since a TD is akin to
doing a "1 0 0 1 x y", where x & y are the params to TD. So whatever
you are doing with them in general in your text calcs, you'd do with
them here.

Are you doing simple algebra, or matrix math? You need to do
a full matrix multiply.
